As you probably know use of medical marijuana in state of Michigan is legal. This doesn’t mean you can go out and buy marijuana on street corner. If you have a medical condition that can be treatable with marijuana you can apply to participate in medical marijuana treatment program. If you have a medical condition that can be treat with medical marijuana here are few things you should know before you apply.
Not all medical conditions belongs into this group. Eligible medical conditions are cancer, Crohn’s disease, positive status for virus (HIV), glaucoma, hepatitis C, immune deficiency syndrome (AIDS), agitation of Alzheimer’s disease and nail patella. In this group of conditions are also all medical condition which treatment produces at least one of the following:
severe nausea, a chronic or debilitating disease, muscle spasms, cachexia or wasting syndrome, severe and chronic pain, seizures (including epilepsy). If think your condition is within these mentioned conditions and you want to apply for medical marijuana use, here is what you have to do. You have to submit these information: name, address, and birth date, name, address, telephone number of your physician, an application or renewal fee, a written certification from your physician that you are qualified patient, if you have a caregiver his/hers name, address, and birth date. There is a certain age limit – you must be 21 or older than that. If you are under 18 you need a parent or guardian consent. If this is your first application you will have to pay a fee which is $100.00. If you can show your qualification from Medicaid Health Plan or you have prescription from SSD or SSI you will have to pay $25.00. In any case you can pay in several monthly installments. According of the Michigan Medical Marihuana Act (MMMA) a written certification from your physician must be new not older than 3 months. Once you have sent your application MMMP will within 15 days review your application and made finial decision. If your application is approved you will received a registry identification card within 5 days. Make sure you send all needed information and documents otherwise your application will be denied. Before you send your application and all needed documents make a copy, just in case if your application get lost you can send it again. You can withdraw from the program in any time you like all you need to do is to send written statement. It is your responsibility to return all cards that are associated with you. If you decide to withdraw from the program before your card is issued you will get your money back. If your card is already issued you can’t get any refund of your money. If you change your address or any data from documentation you send in a first place you must notify MMMP within 14 days. This can’t be done over the phone, once again you have to send the written statement. You will receive new card with new information data. For this service you will have to pay $10.00 fee.
